Dear Patients,

This letter is to inform you that I will be closing my family practice at Caleo Health and soon I will be focusing on frontline emergency care, as well as youth and addiction services. My practice at Caleo Health will be closing permanently effective May 3, 2022. As a result, you will need to find a new family physician.

During this transition period, my family medical practice will be attended to by the Calgary Foothills Primary Care Network (“CFPCN”) and by Dr. Gord Malcolm who is my locum physician at Caleo Health until April 15, 2022. Dr. Gord Malcolm will be available to meet your current primary medical needs.

To arrange an appointment with Dr. Gord Malcolm, you can contact Caleo Health at 403.547.9135.

After April 15, 2022, Caleo Health will continue to assist patients with accessing care through Health Link (811) and the Primary Care Network until May 3, 2022.

I understand that this may not be ideal for some patients, therefore I have provided additional options to assist you with finding a new family physician:

Your existing medical records will remain under the care and custody of Caleo Health, specifically, Dr. Ken Thomas. Should you need access to your medical records for any reason, you may request your medical chart, free of charge, by calling 403.452.0999 extension 113 or by emailing Caleo Health at [email protected].

You may also have an alternative care provider contact Caleo Health to arrange your chart transfer.

As with anyone on the patient panel I attend to, I greatly enjoyed listening to you as a patient and wish you the best. It has been an honour providing you care over the last 6 years.
