Benefits of The Program
If you or your patient has been involved in Motor Vehicle Accident (MVA) understanding your diagnosis and treatment options will allow you to access timely care and the most appropriate care pathway(s). Not all MVA injuries are the same and a comprehensive specialist assessment will allow you and your healthcare provider to have a better understanding of your short-term and long-term treatment needs.
Not Sure What to Do?
If you have been involved in a Motor Vehicle Accident Get PEACE OF MIND, by getting an accident injury assessment done by an MVC clinician prior you starting any physiotherapy or chiropractic treatment.
Have your physician send MVC a referral, we will help you complete the necessary forms and help you navigate the system should you require treatment.
MVC Clinic operates at various rehabilitation locations across Calgary to allow for more convenient access to care. We will assess and provide timely reporting, you and your patient stay well informed of all the necessary treatment options. When indicated, we will work with you to gain quick access to medical opinions through the eMD eConsultation portal, Caleo Health Spinal Triage, and/or a physical medicine consult.

Accident Benefit Forms
If you are hurt in an accident and need to claim the cost of your treatment, you will need to complete and submit a notice of loss and proof of claim form AB-1 and have your healthcare practitioner complete a treatment plan form AB-2 within 10 business days from the date of the accident.
If you are not able to work because of your injuries and want to apply for disability benefits, you must complete the front side of the claim for disability benefits form AB-1A and have your medical doctor complete the other side confirming that you are medically unable to work because of your injuries.
