This letter is to inform you that Dr. Frederick van Rooyen will be closing his family practice.
Dr. van Rooyen has begun the process of closing his family medicine practice effective immediately, you will have until August 15, 2021 (up to 90 days), to find a new family physician.

Caleo Health is pleased to continue providing medical services for you and your family. You will have the option to transfer to Dr. Caeley Lorincz’s practice, effective immediately.
Dr. Lorincz and her team of Nurse Practitioners will be available to meet your medical needs and provide you with uninterrupted healthcare.
