Dear Patients,

I’m writing this letter to inform you that as of April 6, 2020, I will be relocating my practice from the Caleo Health Clinic to the ALGOS Medical Clinic.  The new office location and contact information are as follows:

You may call the new location to book or cancel appointments as of April 6, 2020.  However, we request your patience with any delays as the new staff adjusts to the increase in volume of calls.  In the interim, please continue to book with the Caleo Health clinic at (403) 984-5458

Please note that your records will be securely transferred to the new clinic.  If the new location does not suit your healthcare needs, kindly send me a signed written request to transfer records to another physician of your choice.

I will continue working closely with the physicians, surgeons, physiotherapist and the other healthcare professional at Caleo Health, to ensure a seamless transition.

Dr. Gupta and Dr. Barnard, at Caleo Health will continue to assist patients that choose not to relocate.  Please let me know if you would like a referral to Dr. Gupta or Dr. Barnard for your ongoing care.
